Below you will find the new patient paperwork. Please fill this out completely and bring to your first appointment. If your medication list does not fit in the provided space please write them out on another piece of paper and attach it to the paperwork.  Please bring a photo ID and insurance card (if you are having us bill your insurance).  Please arrive 20 minutes before your scheduled appointment so we can get your chart started.  New Patient PaperWork When you print out the new patient paperwork it will tell you the words are out of the margins, accept it and hit OK. Do not change the margins, it will not PRINT out correctly.        We ask all patients to refrain from wearing any scented lotions and perfumes to your appointment                                                                      Thank you.
